Students requiring any official academic documentation, including but not limited to transcripts, recommendation letters, and evaluation forms, to be sent on behalf of college applications, pre-college programs, scholarships and more must complete this form in its entirety and present it to the College Office in person.
Students requiring a teacher/counselor recommendation letter or the completion of an evaluation form for any reason must complete this form in its entirety, typed and error-free, and present it to the respective teacher/counselor in person a MINIMUM of four weeks prior to the desired mailing date.
All seniors are required to complete one of these Checklist Forms for EACH of the colleges or universities they are actively in the process of applying for. All information marked by an asterisk (*) is to be filled out before turning the form into the College Office together with a 9″x12″ mailing envelope affixed with 4 first-class stamps. The compiling of any required college application materials will not commence until a checklist and envelope are received. Once received, A.U.C.’s College Office will consider the application officially active.
Students experiencing financial hardship may get their college application costs waived. However, not all post-secondary institutions allow students displaying econimic need to forego paying the application fee. Furthermore, colleges that do allow their fees to be waived require specific documentation which may include their own institutional forms, the College Board’s college application fee waiver, the ACT college application fee waiver, or the NACAC application fee waiver. Students must contact the indivdiual college admissions departments to inquire about the specific forms and procedures accepted by each particular insitution. The NACAC application waiver form found below is, however, widely accepted.
College Board & ACT Application Waivers are NOT the same waivers used for the SAT & ACT exams, though these waivers can be picked up in AUC’s College Office as well.
Students requiring either an unofficial transcript for personal review or an official transcript for pre-college programs, scholarship programs, internships, and volunteer programs must complete this form in its entirety, typed and error-free, and present it to the College Office a MINIMUM of two weeks prior to the desired mailing date. Please read and follow carefully the directions included.
Students planning to complete volunteer work should review the rules and procedures PRIOR to commencing their service as explained on page “1” of this packet. Once volunteer work is completed, copies of these forms, including the log sheets completed throughout the duration of the community service work, the reflection form completed after the volunteer work ends, and the official verification letter from the institution where work was completed should be submitted to the College Office.
