PRESEASON CONDITIONING/TRYOUTS & PARTICIPATION REQUIREMENTS (Submit in Person)

In order to condition/tryout and participate in our Athletics' teams students must submit in person the following forms to the ACC office at Building A right after dismissal.

EL1 - ECG Screening form - required form starting July 1, 2026 for all new student-athletes who have not participated in competitive AMC sports the previous year(s). Returning student-athletes are not required but strongly encouraged to obtain an ECG. The goal is to identify undetected cardiac risk factors and enhance the athlete's safety.
ME1 - For use by Medical doctors to exempt a student athlete from the ECG requirement.
Should form ME1 be needed, please be sure to complete student information at top of page. The medical exception determination must be completed by a licensed physician.
EL1/2s - This form is a medical eligibility supplemental form. It should only be used if the student is referred to a qualified medical specialist prior to clearance for participation in sports.
EL2 - The form should be completed, signed and sealed by the health care provider. The form must be signed and dated by the student and parent/guardian.
EL3 - The form must be signed and dated by the student and parent/guardian(s).

INSURANCE POLICY

By selecting this button I agree to hold harmless Archimedean Schools’ Board, the Athletic Department of Archimedean Schools, and the Archimedean Community Center from any direct or implied responsibility for expenses resulting from any before and after school activity injury. The Archimedean Schools’ Board, requires that the parents/guardians of all students participating in the Archimedean extracurricular activities, have medical primary insurance coverage in place for their children on a family plan. The parent/guardian must cover all expenses in the event of an activity-related injury.

COMPETITIVE TEAM PARTICIPATION GENERAL POLICY
In order for the athletic program to maintain competitive athletic teams, parents/guardians of the student-athletes that are selected during try-outs to participate in the AMC athletic teams, are required to contribute to the expenses of the program as they are set by the school’s Administration. The contribution towards the expenses of the athletic team is non-refundable and non-transferable to other teams.

Credit card authorization has been submitted to the Archimedean Cashier at the time of this registration ( a new credit card authorization form shall be submitted every school year).

PARENT/GUARDIAN CONSENT

I have read, understand, and agree with the insurance and the participation policy that Archimedean Schools requires. By signing below I certify that all information I have written on this document are correct and I give my consent for my child to participate in the athletic program of the Archimedean Schools and to be transported by buses, when needed, to and from athletic competitions. I give permission to the Archimedean Athletics’ Staff to take whatever emergency measures are deemed necessary for the care and protection of my child while under their supervision. In case of a medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the local emergency team deems it necessary. It is understood that in some medical situations the Archimedean Athletics’ Staff will need to contact the local emergency resources before the parent/guardian. Finally, I agree to contribute to the expenses of the program as they are set by the school.
